Left paracolic gutter anatomy.

There are two paracolic gutters in a human body. The main paracolic gutter lies lateral to the colon on each side. The left lateral paracolic gutter. The left gutter runs between the descending colon and the abdominal wall and just like the right gutter empties into the lower abdomen and pelvic area.

The main paracolic gutter lies lateral to the colon on each side. The right and left paracolic gutters are peritoneal recesses on the posterior abdominal wall lying alongside the ascending and descending colon. It is smaller than the right paracolic gutter. This gutter is however much smaller because it is restricted at the top by the phrenicocolic ligament or the ligament supporting the top left edge of the colon.

The left and right supramesocolic spaces usually communicate freely with one another and include the perihepatic left subphrenic and perisplenic spaces fig 8a 8b. The right and left paracolic gutters are peritoneal recesses on the posterior abdominal wall lying alongside the ascending and descending colon. The phrenicocolic ligament is a relative but incomplete impediment to the spread of pathologic processes from the left paracolic gutter to the left subphrenic space fig 3. Left lateral paracolic gutter similar to the right the left lateral paracolic gutter extends from the left colic splenic flexure adjacent to the lateral wall of the descending colon and medial to the left lateral part of the anterior abdominal wall to its termination at the at the sigmoid mesocolon s lateral edge.

The left paracolic gutter is a component of the left inframesocolic space partially separated from the left subphrenic spaces by the phrenicocolic ligament. Both paracolic spaces are in continuity with the pelvic peritoneal spaces.

The right and left paracolic gutters are peritoneal recesses on the posterior abdominal wall lying alongside the ascending and descending colon. A less obvious medial paracolic gutter may be formed especially on the right side if the colon possesses a short mesentery for part of its length.

The left lateral paracolic gutter. The right paracolic gutter is a component of the right inframesocolic space continuous superiorly with the right subhepatic and right subphrenic spaces it is larger than the left paracolic gutter which is partially separated from the left subphrenic spaces by the phrenicocolic ligament.
